Welcome to FirstCare's Special Investigations Unit (SIU)
Health care waste, fraud and abuse cost Americans billions of dollars every year. We all pay in reduced benefits, higher taxes, increased premiums, and more expensive doctor visits and prescriptions.
We have established this SIU Online Incident Report Form in order to provide both internal and external sources the ability to report health care fraud, as it relates to FirstCare Health Plan business.
FirstCare's SIU was established to investigate health care fraud and compliance incidents that may have been perpetrated by either a FirstCare member, FirstCare provider, and /or other party.
FirstCare may refer investigative results, including materials from the investigation file, to appropriate law enforcement and regulatory authorities. FirstCare works jointly with various state and federal agencies to prosecute health care fraud cases, including the Federal Bureau of Investigation (FBI), Internal Revenue Service (IRS), United States Health and Human Services Division - Office of Inspector General (OIG), Texas Attorney General's Office, Texas Health and Human Services Commission - Office of Inspector General, and local law enforcement authorities.
Please be advised, any information provided in the FirstCare Incident Report Form will become part of an investigation which may be shared with federal or state government agencies. Generally, information received by FirstCare's Special Investigations Unit is considered confidential and will not be disclosed unless otherwise required by law.
