FirstCare (PPO) Plan

With FirstCare PPO, you do not have to choose a Primary Care Physician.  You may decide whether you will seek care from an in-network preferred provider, or from an out-of-network non-preferred provider.

FirstCare PPO gives you the ability to choose from one of four different individual benefit plan options.  These plans give you the flexibility to:

• Select what annual deductible and coinsurance amounts best fit your individual needs.
• Determine whether or not you need coverage for prescription drugs or maternity benefits. 
• Decide the premium payment you can afford and you design your plan to meet your personal budget.

Through our extensive network of participating providers, you and your dependents have continuous access to quality health care services.
