FirstCare Advantage Elite has all of your coverage in one place, including Medicare Part D prescription drug coverage. FirstCare Advantage Elite has comprehensive benefits with minimal cost-sharing to you.
FirstCare Advantage Platinum (HMO)
FirstCare Advantage Platinum (HMO)has the same great benefits as FirstCare Advantage Elite (HMO), however, the premiums are lower with similar cost sharing or many benefits to you. Also included in the FirstCare Advantage Platinum is coverage for Medicare Part D prescription drugs.
FirstCare Advantage Gold (HMO)
With FirstCare Advantage Gold (HMO), you get all the benefits available under Original Medicare and more, which includes Medicare Part D prescription drugs.
FirstCare Advantage Silver (HMO)
The same great benefits as FirstCare Advantage Gold (HMO), without the Part D drugs. This plan is designed for people who don't need or want drug coverage but still want to take advantage of all the benefits of FirstCare Advantage.
FirstCare Advantage Select - for individuals who are dually eligible for Medicare & Medicaid coverage.
This plan is designed to meet the special needs of those who are eligible for both Medicare and Medicaid and live in the Lubbock region. You must be eligible for both in order to enroll in Advantage Select. Enrollment for the Select plan Is available anytime of the year.
