• Seven plan designs available, including HMO, PPO, and a Covenant-only plan
• Dual-option plans

Eligibility Requirements

To be eligible for the chamber plan, an employer must

• Be a member of the Lubbock Chamber of Commerce
• Operate in FirstCare's service area
• Have a minimum of two verifiable employees
• Cover at least 75% of employee-only premium
• Have 75% of eligible employees participating
