FirstCare Individual & Family Plans
Effective July 1, 2010, our Individual plans are no longer accepting new applicants unless they are an eligible dependent of an insured that is currently enrolled in one of our MySeries plans.

Our ChoicePlus plan provides comprehensive coverage for preventive and emergency care, including prescription coverage and out-of-network benefits. This traditional preferred provider organization (PPO) plan offers a freedom of choice among in-network providers and a maternity rider option.

SelectHC is designed to be compatible with your Health Savings Account (HSA). An HSA is a separate bank account designated for pretax money used to pay qualified medical expenses. The plan includes pharmacy benefits and the deductible is waived for preventive care.

MyHMO is a health maintenance organization (HMO) plan that includes preventive, inpatient, outpatient, and emergency care, including prescription coverage. Your out-of-pocket costs are lower and more predictable than with other types of plans.

Effective July 1, 2009, our 2004 individual PPO plans are frozen. New applicants cannot enroll under the existing PPO plans unless they are a dependent of an insured that is currently enrolled in one of the frozen plans.
