FirstCare Advantage (HMO) is a health plan with a Medicare contract. Enrollment in FirstCare depends on contract renewal. If Medicare determines during the year that our contract must be terminated, then the plan's members will be notified in advance of the plans termination. Enrollment is limited to certain times of the year. Please see eligibility restrictions for more guidance.
If FirstCare Advantage (HMO) terminates the contract with CMS, a notice to all members will be provided within 60 days prior to the termination. If CMS elects to non-renew the FirstCare Advantage (HMO) contract, a notice to all members will be provided within 30 days prior to the termination. The notice will include a list of alternatives available in the area for obtaining Medicare services. In addition, FirstCare Advantage (HMO) will publish a CMS approved notice of the contract termination/non-renewal in the local newspapers.
