The rising costs of health care and prescription drugs are sending medical expenses higher and higher. FirstCare Advantage (HMO) was created for the needs of West Texans. In partnership with Medicare, our plans are designed to offer affordable, flexible and simple plans to understand and use. We provide emergency coverage anywhere in the US and access to a large network of doctors and hospitals providing the best care available.

The links in the left-hand navigation will lead you to valuable information about our plan, including formularies, provider/pharmacy directories, frequently asked questions, and enrollment instructions.

Eligibility

You qualify to become a FirstCare Advantage (HMO) member if:
  • You are entitled to Medicare Part A, enrolled in Part B, and continue to pay your Part B premium if not otherwise paid by a third party.
  • You live in the counties:
Lubbock Region: Crosby, Floyd, Garza, Hale, Hockley, Lamb, Lubbock, Lynn, or Terry
Midland/Odessa Region: Ector or Midland
Abilene Region: Jones or Taylor
 
  • You do not have End Stage Renal Disease at the time of enrollment (certain exceptions apply).
We offer a variety of plans, all of which provide the benefits of the Original Medicare Plan, plus additional benefits not offered by Medicare.

FirstCare Advantage (HMO) is a Health Plan with a Medicare Contract, which renews annually.  Availability of coverage beyond the end of the year is not guaranteed.  If Medicare determines during the year that our contract must be terminated, then the plan's members will be notified in advance of the plans termination.  You must be entitled to Medicare Part A, be enrolled in Part B, and reside in our service area to be eligible.  Enrollment is limited to certain times of the year.  Please see eligibility restrictions for more guidance.
 
If FirstCare Advantage (HMO) terminates the contract with CMS, a notice to all members will be provided within 60 days prior to the termination. If CMS elects to non-renew the FirstCare Advantage (HMO) contract, a notice to all members will be provided within 30 days prior to the termination. The notice will include a list of alternatives available in the area for obtaining Medicare services. In addition, FirstCare Advantage (HMO) will publish a CMS approved notice of the contract termination/non-renewal in the local newspapers.

FirstCare Advantage (HMO) has been approved by the National Committee for Quality Assurance (NCQA), a non-profit organization dedicated to improving health care quality until 12/31/2017.

Am I in the FirstCare Service Area?





NCQA awards a status of Interim to organizations with basic structure and processes in place to meet expectations for consumer protection and quality improvement. Organizations awarded this status will need to undergo a new review within 18 months to demonstrate they have executed those processes effectively.
