- Sign up for our Disease Management Program
To ensure your ongoing good health, FirstCare provides a Disease Management Program. Our Disease Management covers asthma and diabetes. This program is an integrated, comprehensive, and customized approach for you and may be accessed on the internet for your convenience. Through integration of selection criteria and medical claims data, a comprehensive clinical profile is provided.
Members receive relevant information to keep up to date on a particular disease and information important in maintaining good health and wellness. This places you “in the know” to better manage your overall health.
Participation in the FirstCare Disease Management Program is optional. By signing up for our Disease Management Program, the information you provide will be incorporated into the FirstCare Health Plan Disease Management Registry. You may also request that additional information concerning your health and well being be provided to you.
