Transitional Care Benefit
With FirstCare, all care must be properly referred by your PCP. If you are undergoing a course of treatment at the time of your effective date, and the treating physician is not a participating FirstCare provider, one of the following will take place:
- Your PCP and FirstCare will transition your care to an in-plan provider if at all possible; or
- If your treatment or condition is such that your care cannot be transitioned to an in-plan provider, you may request ongoing care with your current provider for a period of time. FirstCare may authorize the use of an out-of-plan provider.
