  The National Diabetes Information Clearinghouse (NDIC) is an information dissemination service of the National Institute of Diabetes.
• Sign up for our Disease Management Program
  By signing up for our Disease Management Program, the information you provide will be incorporated into the FirstCare Health Plan Disease Management Registry. You may also request that additional information concerning your health and well being be provided to you.
