2006 FirstCare Disease Management Program (DMP)

FirstCare has had a Disease Management Information Program for Asthma and Diabetes since September, 2003.

The DM program provides a comprehensive informational approach to disease management via the WEB. This WEB site enables FirstCare to:

• Proactively inform members about Asthma and Diabetes
• Potentially increase population-based clinical guideline compliance
• Support sustained behavior change

The following chronic disease states are offered:  Asthma and Diabetes.

Through WEB based education members receive information that increases the participant's knowledge of their appropriate disease state.  The goal of this information is to develop and facilitate behavioral change, while supporting members with a focus on learning self-care skills. 

By empowering patients to better manage their chronic conditions, FirstCare anticipates that:

• Improvements in self reported quality of life indicators could be measured
• Increased compliance with prescribed treatment regimens and disease specific national guidelines/recommendations may occur
• A decrease in adverse clinical symptoms is potentially possible
• Cost savings through appropriate utilization of medical resources may occur
• Higher Satisfaction may be reported by members
