FirstCare Advantage Dual SNP (HMO SNP) is a Health plan with a Medicare contract and a contract with the Texas Medicaid program. Enrollment in FirstCare Advantage Dual SNP (HMO SNP) depends on contract renewal.
 
If FirstCare Advantage Dual SNP (HMO SNP) terminates the contract with the Centers for Medicare and Medicaid Services (CMS), a notice to all members will be provided within 60 days prior to the termination. If CMS elects to non-renew the FirstCare Advantage Dual SNP (HMO SNP) contract, a notice to all members will be provided within 30 days prior to the termination. The notice will include a list of alternatives available in the area for obtaining Medicare services. In addition, FirstCare Advantage Dual SNP (HMO SNP) will publish a CMS approved notice of the contract termination/non-renewal in the local newspapers.
